Online Registration Form

Important Instructions for Parents/Guardians:

1. Parents/Guardians are required to complete the Online Registration Form available on the school’s official website, www.dpsgaya.com, ensuring that all information provided is accurate and complete.

2. After successfully submitting the Online Registration Form, parents/guardians must print the completed form and affix recent passport-size colour photographs of the candidate, father, and mother in the designated spaces.

3. The printed Registration Form must be submitted in person by the parent/guardian at the school office, along with the following documents:

• A self-attested photocopy of the candidate’s Birth Certificate.
• The confirmation receipt of online payment of the Registration and Processing Fee of ₹2,500/-, if paid online.

4. The Registration and Processing Fee of ₹2,500/- may be paid online or in cash at the school’s designated facilitation counter.

5. You must complete all registration formalities, including submitting the printed Registration Form, supporting documents, and paying the prescribed fee, within three days of submitting the Online Registration Form.

Important Terms and Conditions

• The Registration Form must be completed carefully. Any discrepancy between the information provided in the form and the supporting documents may result in rejection of the application.

• Submission of multiple Online Registration Forms for the same candidate may lead to cancellation of the registration.

• Incomplete applications or failure to complete the prescribed formalities within the stipulated period may result in rejection.

• The school’s decision regarding the acceptance or rejection of registration applications shall be final.

• Submission of the Registration Form does not guarantee admission. Admission is subject to the prescribed admission process and seat availability in the respective class.

IMPORTANT NOTICE

Online submission of the Registration Form alone does not complete the registration process. Parents/Guardians must submit the printed Registration Form, required documents and prescribed fee at the school office within three days of online registration.
